if you fish very much, twice a year is a must. that lower unit lube is sure a lot cheaper than a new gear case. and the plus side of doing it twice a year, is that sometimes you can catch a problem early, and the fix can be very minimal compared to replacing the lower unit.
I recommend changing it once a year in the fall. If you have had no lower unit trouble that was apparent and you have water in it (milky) or metal chunks, that is the time to do something about it before the LU freezes and cracks, or you can fix your gears when you aren't fishing.
